Form to declare if crew onboard a vessel on a selected date holds Crew Effects to be reported for customs control

Use selected crew change - recommended to set this checkbox when report is run from the Crew Change module. Date & Vessel are then disabled for input, being defined from active module's record
Vessel Pool - to select from Companies that possess vessels from Organization Structure
Vessel - list of non-passive vessels from selected Pool
Arrival Date - ETA of a calling port to be verified
Sort By - to define crew sorting order
Crew Effects - to limit with certain codes only
