The report is a fixed layout element designed to suit the pre-defined positioning of the I-95 form of the US Department of Homeland Security.

Use Selected Crew Change - for the report to be run from the Crew Change module allowing to process debarking/embarking/onboard crew only
Crew Selection - to be run for certain crew list members only
Layout - allows customizing the positioning of layout elements if certain printer settings differ from those used by default
Form Height - to customize form height, the value should be defined in pix
Address in United States - free input text to appear in the Home Address section (not viewed in the report, in designer mode only)

The vessel name is shown only if non-planned sea-service activity crossing the current date exists.
